McDowell Local Food Advisory Council is one of more than 50 Human Service Organizations (HSO) participating in the Healthy Opportunities Pilot (HOP) in Western North Carolina.
HOP is the nation’s first comprehensive program to use Medicaid funding to
support nonmedical health-related needs for Medicaid members who could benefit the most.
Through HOP, eligible participants can receive services at no cost to them to support needs related to food,
housing, transportation, personal safety and healthy relationships.
To be eligible for pilot services, individuals must:
-Be enrolled in an NC Medicaid Managed Care health plan
-Live in one of the state’s three pilot regions, including WNC
-Have a qualifying physical or behavioral health condition, and
-Have a qualifying social need or risk factor.
As an HSO, McDowell LFAC supports HOP participants through available HOP services.
We can help HOP participants access other needed services such as diabetes prevention education,
medically tailored meals, affordable and safe housing, transportation or vehicle repairs, parenting
education and personal safety support.
