We are aware that news is circulating regarding the conclusion of Healthy Opportunities Pilot (HOP) services on July 1, 2025. We want to assure our community that we have been informed and are closely monitoring this evolving situation.

Care managers will be reaching out to participants with additional information. If you are currently enrolled in HOP, please direct any questions or concerns to your care manager, who remains your primary point of contact.
